Position Summary:

Shawnee Christian Healthcare Center seeks a Full-time Check Out Coordinator to participate as strong team member of the SCHC front office staff.

Essential Duties and Responsibilities:

  • Answer phones.
  • Schedule appointments.
  • Check patients out upon appointment completion.
  • Ability to maintain an effective, positive working relationship with co-workers, patients, and the public.
  • Greet patients and visitors in prompt and courteous fashion.
  • Collect and update patient demographic and insurance information and make updates in the Electronic Health Record.
  • Collect insurance co-payments and balances from patients.
  • Perform related work as required.

Job Qualifications and Requirements:

  • Minimum of High School Diploma.
  • At least three years of work experience, preferably in a medical office setting.
  • Knowledge of business office procedures, ability to speak clearly and concisely, ability to read and understand written instructions.
  • Experience with Electronic Health Records and Patient Accounts Receivables preferred.

 Position Summary:

This position supports the Patient Centered Medical Home model by working with community members to increase access to affordable healthcare. The Outreach Worker conducts outreach and educational activities with an emphasis on assisting community members in enrollment in health insurance options. The Outreach Worker will be an integral member of the patient engagement and Neighborhood Transformation team.

Essential Duties and Responsibilities:

  • Conduct outreach activities at local and regional events to educate community members.
  • Participate in training provided by the state and federal government related to enrollment in Kentucky HEALTH, healthcare.gov and pass applicable certification exams.
  • Participate in Medicare and Social Security training in order to guide individuals through these systems.
  • Conduct training for health center staff and other agency staff as requested.
  • Assist qualified persons in enrollment and/or re-enrollment in health insurance options in-person and by phone.
  • Accept referrals for assistance with enrollment and re-enrollment.
  • Maintain all required documentation and submit monthly reports detailing outreach and enrollment efforts.
  • Travel to consumer locations to assist with enrollment activities (i.e., home visits, other agencies, churches, etc.)
  • Assist consumers in the appeals process when needed.
  • Maintain a high level of consumer satisfaction.
  • Assist consumers with referrals to health care resources within their community.

Job Qualifications and Requirements:

  • Minimum of Associates degree in human services; Bachelor’s in social work or related field preferred.
  • Excellent interpersonal skills, including strong verbal and written communication skills.
  • Competent in use of software including Excel and database management.
  • Competent in use of social media.
  • Excellent organizational skills.

Duties and Responsibilities:

  2. Files all medical records in appropriate order and section, and does so in a timely manner.
  3. Provides excellent service to patients and customers by answering phones with appropriate etiquette.
  4. Answers telephone, screens calls, takes messages, and provides information.
  5. Assist the Medical Receptionists in maintaining a neat and orderly work area and lobby.
  6. Monitor fax machine throughout the day and assure faxes get to appropriate personnel.
  7. Performs related work as required.

Performance Requirements:  Knowledge of business office procedures.  Knowledge of grammar, spelling, and punctuation to type patient information.  Skill in operation of a computer and other office equipment.  Skill in greeting patients and answering the telephone in a pleasant and helpful manner.  Ability to speak clearly and concisely.  Ability to read, understand and follow oral and written instructions.  Ability to sort and file materials correctly by alphabetic and numeric systems.  Ability to establish and maintain effective working relationships with patients, employees and the public.

General Summary of Duties:  The Expanded Duties Dental Assistant will support the dentist in providing high quality oral health care and education to a broad range of patients. This position also offers assistance where needed to ensure smooth, efficient running of the dental program and participates in ongoing evaluation of operations to assure delivery of outstanding patient care as well as financial sustainability of the clinic.

Supervision Received: In conjunction with Dental Office Coordinator

Essential Duties and Responsibilities:

  1. Prepares and maintains dental instruments, supplies and equipment, including taking inventory and reordering as necessary
  2. Maintains proper aseptic techniques necessary with dental equipment; cleans and properly sterilizes instruments and records all sterilization activities; helps manage the dental office infection control plan to meet OSHA, CDC and ADA standards
  3. Prepares the operatory for patient visits, including cleaning, disinfecting and restocking the room
  4. Scans reception room for patients and helps to manage patient flow
  5. Seats patient in chair and explains procedures to be performed; Collects and records patient health histories
  6. Prepares dental materials cements, amalgams, composites, impression materials, etc.
  7. Provides active chair side assistance; transfer instruments as appropriate for four-handed dentistry; anticipates the dentist’s needs
  8. Monitors and documents the flow of laboratory cases; maintains lab facility
  9. Takes and prepares radiographs using conventional or digital technology and following proper radiation safety precautions
  10. Applies topical fluoride and varnishes, and pit and fissure sealants
  11. Polishes coronal surfaces of patient’s teeth (without high-speed instruments) to remove stains/soft deposits and polishes amalgams or composite surfaces
  12. Fabricates, preliminarily fits and removes temporary crowns for final cementation; places temporary restorative materials and removes excess cement from fabricated restorations
  13. Takes impressions for study casts, athletic mouth guards and custom trays. Performs laboratory procedures neatly and efficiently
  14. Supports and assists special community oriented dental care programs.
  15. Performs related work as required.
  16. Adheres to SCHC Statement of Faith.

Performance Requirements: Strong communication and interpersonal skills, respect for and appreciation of diversity, cultural competence, ability to operate all types of dental equipment and machinery, able to maintain patient records in a timely manner. Skill in operation of computer and other office equipment. Ability to speak clearly and concisely. Strong organizational skills and ability to manage multiple tasks to completion. Ability to establish and maintain effective working relationships with patients, employees, and the public.  Comprehensive knowledge of dental assisting techniques and procedures for all phases of general dentistry, knowledge of general materials, their storage, handling and application is required.

Education: High school diploma or GED certificate, graduate certificate from an accredited course in expanded duties dental assisting

Experience: At least 5 years of experience providing services in a clinic setting preferred.

 Position Summary:

General Statement of Duties:  Perform accounting duties as directed by the CFO.  Requires strong computer skills with various software programs, especially accounting related.  Ability to communicate effectively with all levels of staff as well as patients and outsider vendors.  Sensitivity to confidential matters is required.

Supervision Received:  Reports to CFO.

Supervision Exercised:  None.

Duties and Responsibilities:

  1. Perform tasks and special projects assigned by the CFO.
  2. Basic accounting and general ledger functions using QuickBooks or similar accounting program.
  3. Assist the CFO with reconciliations of various accounts.
  4. Perform Accounts Payable and Accounts Receivable functions as directed by the CFO.
  5. Assist the CFO in maintaining financial records in accordance with generally accepted accounting principles.
  6. Prepare reports for the outside auditors and assist the CFO with the audit to assure that the audit is performed in accordance with general accepted auditing standards.
  7. Assist with the transmission of payroll to an independent payroll company to ensure timely and accurate payments to employees and taxing agencies.
  8. Assist with HR functions.
  9. Maintain accounting related files.
  10. Perform related work as required.
  11. Adhere to SCHC Statement of Faith.

Performance Requirements:  Skills include but are not limited to:  Pleasant personality, cooperative attitude, written and oral communication skills, organization skills and accounting skills.  Proficient in computer applications – mainly, Word, Excel, and other office related programs.  Basic accounting skills and experience with QuickBooks or similar accounting program strongly preferred.

Position Summary:
A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. A CHW assists individuals and communities to adopt healthy behaviors. Conducts outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health. May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs, and provide services such as first aid and blood pressure screening. Will collect data to help identify community health needs.

Essential Duties and Responsibilities:

  • Convey the purposes and services and impact of program to the user population
  • Help patients develop health management plans and goals
  • Follow-up with health management/care plans with both patients and providers
  • Coach patients in effective management of their chronic health conditions and self-care
  • Assist patient in understanding care plans and instructions
  • Document activities, service plans, and results in an effective manner while strictly adhering to all policies and procedures
  • Work collaboratively and effectively within a team
  • Establish positive, supportive relationships with participants and provide feedback
  • Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
    Assist clients in accessing health related services, including but not limited to:

    • obtaining a medical home
    • providing instruction on appropriate use of the medical home
    • overcoming barriers to obtaining needed medical care and social services
  • Facilitate communication and coordinate services between providers
  • Motivate patients to be active, engaged participants in their health
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
  • Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff
  • Continuously expand knowledge and understanding of:
    • community resources, services and programs provided;
    • human relations and the procedures used in dealing with the public as part of a service or program;
    • volunteer resources and the practices associated with using volunteers;
    • operations, functions, policies and procedures associated with the department  or program area;
    • procedures and resources available to handle new, unusual or different situations
  • Identify and apply appropriate role definition and skilled boundaries
  • Collect data and participate in data and outcomes management strategies
