About the Pharmacy

After your visit with your SCHC provider, stop by the pharmacy to check the status of your prescription. Most prescriptions will be ready within minutes of your completed medical or dental visit here at the center.

Patients will need to bring their prescription insurance cards. We accept Medicaid, Medicare Part D, and most commercial insurances. We work directly with our patients to provide them with their medication at the lowest possible price. We have a sliding fee scale for pharmacy patients who have completed the application and are verified. To qualify for the sliding fee pricing see the front desk for an application and bring proof of income including: 

• Previous year’s W2 form or tax return

• Current paycheck stubs (at least 3)

• Letter from employer (on company letterhead)

• If self-employed, a Form 1040 tax return from the previous calendar year

• If state income, a letter from a social worker, a copy of an award letter from the SSA, a check stub or a letter with details of pension benefits
