  • We do ask that you make an appointment by calling (502) 778-0001. We do have limited same-day appointments and may be able to accommodate walk-ins, but it is not guaranteed. If you are having an emergency, you should call 911 or go to the nearest emergency department.

    If we are closed, call (502) 778-0001 for the physician on call or go to the nearest immediate care center.

  • In order to have labs drawn, you must be seen by a provider first. To schedule an appointment, please call (502) 778-0001.

  • If you need urgent dental care, you can call the Dental Clinic to request a same-day appointment at (502) 774-2400. We recommend that you call as early in the morning as possible. Availability is not guaranteed, but we will get you in if we can. Same-day appointments are available for all appointments, but those with urgent needs will get priority.

  • For a prescription refill, you can call the pharmacy at (502) 632-1729 or fill out our online prescription refill form.

    Some prescriptions require that you be seen by a provider every three to six months.

  • Need insurance? We can help. Call 502-778-0001 and ask to speak with a Community Health Worker.

    We can assist you with enrolling in Medicaid and other health plans!

  • In addition to accepting Medicaid and other forms of insurance, SCHC has a sliding fee scale to make insurance more affordable to people with low-incomes. No one will be turned away if they can’t pay. To apply for the sliding fee, you should bring proof of income including:

    Previous year’s W2 form or tax return

    Current paycheck stubs (at least 3)

    Letter from employer (on company letterhead)

    If self-employed, a Form 1040 tax return from the previous calendar year

    If state income, a letter from a social worker, a copy of an award letter from the SSA, a check stub or a letter with details of pension benefits

    The poverty guidelines are updated periodically in the Federal Register by the US Department of Health and Human Services under the authority of 42 U.S.C. 9902(2).

  • SCHC serves all patients regardless of their ability to pay and does not discriminate because payment for services would be made under Medicare, Medicaid, or CHIP. SCHC does not discriminate on the basis of race, color, national origin, disability, age, sex, religion, or other legally protected category.

  • You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

    As of January 2022, under a new law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

    • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

    • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

    • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

    • Make sure to save a copy or picture of your Good Faith Estimate.
